Achieve Health is a provider owned healthcare clinic providing Immediate care, Family Medicine, LGBTQ Health, Women’s Health, Hormone Therapy, Psych Support Services and IV Therapy all under one roof. We believe the approach to Achieve Health is a collaborative provider – patient experience focusing on the highest quality of care in a judgement free environment.

Ready to Book an Appointment?
Book your Appointment today. Fees are based on your insurance coverage. We accept all major insurance plans including Medicare and Medicaid. Copays are due at the time of your visit. If you are paying out of pocket, the fee for IMMEDIATE CARE or NEW PATIENTS is $125. EXISTING PATIENTS and FOLLOW UP VISITS $100 and SPECIALTY SERVICES $100 to $125 based on the service. Payment is due at the time of service. We accept all major credit and debit cards. For cash payments we accept Venmo, PayPal or Square.
